Subacute Rehab
Our subacute rehab team include physicians, specialists, nurses and therapists who specialize in a wide variety of conditions. We work to get you home as quickly as possible, without any recurrence or need for a return to the hospital.

On-Site Physiatry
Our experienced Physiatry Team provides a full spectrum of care – from diagnosis to treatment and rehabilitation – to restore maximum health and quality of life. By working collaboratively with our staff, patients and families, they are able to enhance functional outcomes, clinical quality measures, and the patient care experience – ultimately getting patients back to the lives they love.

Wound Care
Supervised by top physicians and specialists, we ensure safe wound care and personal guidance to prevent infection, recurrence, or excessive scarring. We also have a full-time wound nurse and a CRNP who make regular wound rounds. Our expertise in wound care enables patients to transition home easily and complete their recovery process independently.

Long-Term Care
Our long-term care model centers on excellence, with a focus on the individual needs of each resident. From the beginning, we ensure that the transition to Havens living is smooth and successful, maintaining open communication with loved ones and the medical team throughout.

Memory Care
Raising the bar in memory care​​
Our memory care program maintains a familiar environment for our residents. Each day is similar, with predictable activities. We promote independence with routines of daily living. Our residents come to know the faces of our compassionate staff, who are kept on a consistent schedule. Every staff member that works on our memory care unit is highly trained in dementia and memory disorders. We provide them with regular professional development to expand their expertise.

Hospice & Palliative Care
Hospice Care
When you become eligible for hospice care, the medical team is no longer looking for a cure or treatment, but to limit pain and increase your comfort. The Havens vision of hospice care means that every person with a life-limiting illness should spend the time they have left pain-free and surrounded by loved ones, in a space in which they feel comfortable and cared for. Our hospice nurses and physicians are on hand around the clock to address medical needs, answer questions, and offer support.

Palliative Care
Like our hospice unit, palliative care at Havens centers on our goal of maintaining the highest quality of life and comfort for our patients. But unlike hospice, which is centered on end of life, palliative care is available for those with chronic conditions such as Stage 4 cancer, COPD or dementia, who are receiving aggressive treatment for illness, or who may simply need palliative support for a challenging condition. Receiving palliative care does not necessarily mean that you will eventually transition to hospice care; in fact, it is often a support for treatment that can significantly extend your life. Other times, it may be the last stage before a terminal diagnosis. In either case, Havens at Pensacola offers comprehensive palliative care that addresses the needs of body, mind, and spirit.

We offer a wide range of medical treatments as well as support with everyday tasks such as bathing, eating, dressing, and taking medications. At Havens Pensacola, we have structures in place to ensure a smooth, easy transition.

Our dedicated subacute rehab unit is designed to help you recover stronger and return home with confidence. In the Haven's comfortable and welcoming setting, we provide comprehensive, short-term rehabilitation focused on restoring strength, mobility, and independence after surgery, illness, or hospitalization.
Our interdisciplinary team of physicians, nurses, and licensed therapists specializes in treating a wide range of complex conditions.
Through individualized care plans and daily physical, occupational, and speech therapy, we use evidence-based practices and advanced technology to maximize progress and reduce the risk of rehospitalization.
To ensure seamless, coordinated care, onsite specialists regularly round, including a respected area cardiologist, pulmonologist, and nephrologist. Every aspect of your recovery is thoughtfully managed so your transition from hospital to home is smooth, safe, and successful.

Memory Care
We are proud to offer a 15-bed secure unit for individuals with Alzheimer's or other forms of dementia.
Our memory care neighborhood provides a safe, familiar environment where residents benefit from a predictable daily routine that promotes comfort, confidence, and independence. Consistent staffing allows residents to build trusting relationships with caregivers who are specially trained in dementia care and receive ongoing professional development.
Support includes 24-hour skilled nursing, medical oversight for concurrent conditions, and careful medication management.
Residents also enjoy engaging activities, assistance with daily living, and access to social workers and counselors, while families are supported through guidance and dedicated support services.

Wound Care

Our specialized infectious disease program provides expert care for patients requiring advanced treatment and close medical monitoring.
Working in collaboration with infectious disease physicians, our clinical team delivers comprehensive management of complex infections, including IV antibiotic therapy and wound care, in a safe and closely supervised setting.
With 24-hour skilled nursing, careful medication management, and strict infection control protocols, we focus on promoting healing while preventing complications or hospital readmission. Each care plan is individualized, coordinated, and designed to support a smooth recovery and successful transition to the next level of care.
